It could be my own confusion, but it seems to me that The Doctor himself
firmly rejects the idea of describing Hurt's character as "The Doctor",
but the text on the final scene does precisely that.
The Doctor describes his adoption of "The Doctor", and his decision to
behave in a way that matches that description, as a reaction to what
Hurt's character did. This implies that Hurt's character preceded the
Doctor's first incarnation. If they are indeed the same person, that
implies that The Doctor's 11th incarnation is actually that person's
12th incarnation.
I've never read Lungburrow, but I've read about it at <
http://tardis.wikia.com/>. According to that novel, Time Lord Society
was founded by three entities known as Rassilon, Omega, and The Other.
It also says that no new Gallifreyans have been born since they were hit
by the Curse of Pythia, and they now reproduce by a mechanism call a
"Loom". It implies that The Doctor's first incarnation was the
re-Looming, 10 million years later, of The Other. Given how The Other's partners turned out, I could easily imagine that The Other might have
done something that would make The Doctor feel guilty.
I don't consider the novels canonical, but it's possible that the TV
show is planning to use at least some of the ideas from that story.
There's now way to know until November.
